Community Health Development, Inc.
Community Health Development, Inc. (CHDI) was founded in 1983 to increase access to primary healthcare services for the people of Uvalde, Real, Edwards, and Zavala Counties. CHDI is a not-for-profit community health center that annually delivers care to over 13,000 individuals. Our mission is to continuously improve the health and well being of the community we serve. We fulfill our mission by providing high quality medical, dental, and behavioral health care services to all. We are recognized by the National Committee on Quality Assurance as a Patient Centered Medical Home meaning that we provide health care that is relationship-based with an orientation toward the whole person. Partnering with patients and their families requires understanding and respecting each patient’s unique needs, values, and preferences. The medical home practice actively supports patients in learning to manage and organize their own care at the level the patient chooses.
In 2026, CHDI was awarded multiple Community Health Quality Recognition (CHQR) Badges by the Health Resources and Services Administration (HRSA), honoring our continued commitment to accessible, high-quality care.

About Community Health Centers:
Building Care WITH You
At Community Health Centers (CHC), care starts with listening. We take time to get to know our patients – their lives, their needs, and their goals – and we build our care around that.
This approach is about something bigger. As the largest primary care network in the country, CHCs are united by a shared commitment to care that’s grounded in relationships. That’s why the care people receive here feels familiar and personal. It’s care that’s shaped by trust, built on connection, and delivered by a national movement rooted in every community we serve.
