Patient Services Specialist
Location: Uvalde
Position Summary:
This position performs registration of patients seeking services from CHDI and is responsible for greeting, screening, and directing patients to appropriate services. Position performs routine clerical duties following established procedures.
Qualifications Required:
- High school diploma or GED required.
- Experience in a healthcare setting preferred.
- At least one-year of experience answering telephone and working with the public.
- Computer literacy preferred.

Quality Improvement Manager
Location: Uvalde
Position Summary:
Consistent with Community Health Center requirements, is responsible for creating and implementing an annual plan for CPI review, for conducting reviews, creating reports, making recommendations regarding risk management policies and procedures and implementing training based on those reviews, and for taking complaints regarding compliance, investigating and making recommendations for actions based on the results of the investigation. Leads improvement activities pertinent to clinical quality performance and regulatory compliance; applies critical thinking and health center expertise in both assessing and responding to opportunities for improvement; is experienced in data analysis and quality improvement methodology, with skill in taking a collaborative team-based approach to targeted project management. Serves as a liaison to the National Committee for Quality Assurance (NCQA) and is responsible for CHDI maintaining its PCMH status. Serves as CHDI’s Risk Manager, Compliance Officer and Lead Safety Officer. Also responsible for oversight of Health Records Department for maintaining the content of Health Records in accordance with CHDI Policies and Procedures.

Care Coordinator

Position Summary:
Responsible for deliberately organizing patient care activities and sharing information among all of the participants concerned with a patient’s care to achieve safer and more effective care. The main goal of care coordination is to meet patients’ needs and preferences in the delivery of high-quality, high-value health care. The care coordinator has regular contact with patients and documents all activities in the patient’s electronic health record in order to track treatment adherence and response. Certified Medical Coder
Location: Uvalde
Position Summary:
Responsible for reviewing medical documentation ensuring accurate diagnosis and procedure codes. Performs audits on medical records to assess accuracy and completeness of documentation, adherence to coding rules and regulations, and compliance with reimbursement requirements. Serves as expert resource regarding claims issues related to coding, denials, and insurance eligibility.
- Assign codes to diagnoses and procedures, using ICD (International Classification of Diseases) and CPT (Current Procedural Terminology) codes.
- Follows up with the provider on any documentation that is insufficient or unclear.
- Communicates with other clinical staff regarding documentation.
- Communicates and works with Revenue Cycle department to resolve billing issues.
- Researches and analyzes data needs for reimbursement.
- Ensure that all codes are current and active.
- Analyzes medical records and identifies documentation deficiencies.
- Reviews and verifies documentation supports diagnoses, procedures, and treatment results.
- Identifies diagnostic and procedural information.
- Assigns codes for reimbursements, research and compliance with regulatory requirements utilizing guidelines.

Certified Fitness Trainer

Position Summary:
Fitness trainers instruct individuals about exercise activities that improve strength, cardiovascular endurance, and flexibility. They meet with members to understand the member’s personal fitness goals, then develop an exercise program, tailoring workouts to member needs. They demonstrate movements, ask clients to repeat them while offering tips on improving form, and motivate members to return to classes on a regular schedule so physical conditioning can continue steadily. The Fitness Trainers explain safety rules and educate members on the proper use of exercise equipment. They may also give information on nutrition, weight control, and general lifestyle changes.
Requirements Required:
- Strong working knowledge of fitness training and exercises

Registered Dental Hygienist
Location: Uvalde and Leakey
Position Summary:
The direct provision of oral hygiene and appropriate preventive services to dental patients of the agency. The Registered Dental Hygienist will also be responsible for providing appropriate oral health information to individuals and groups.
Requirements Required:
- Possesses a current, valid license as a Dental Hygienist in the State of Texas and provide Basic Life Support as required by the American Heart Association.

Registered Dental Assistant
Location: Uvalde
Position Summary:
Performs routine chair-side dental assisting duties and other related duties involved in providing oral health patient care in one of the Agency’s primary health care clinics. Works under the direction of agency’s dentist(s), registered dental hygienist(s) and the supervision of the Chief Dental Officer. The dental assistant will provide services directed toward the restoration of health and wellness, health promotion and disease prevention.
Requirements Required:
- Certified by the State of Texas as a Registered Dental Assistant.
- Maintains continuing education requirements as mandated by the Texas State Board of Dental Examiners for Registered Dental Assistants licensure and provider Basic Life Support as required by the American Heart Association.
- Experience in the sterility of all reusable dental instruments, devices and equipment in accordance with Dental Department directive and protocol.
- Experience in preparing the patient for treatment including the identification of the chief complaint, the taking and recording of vital signs, weight, temperature, blood pressure, glucose levels, etc., and ensures smooth patient flow throughout the center.
- Knowledge of modern dental material, their storage, handling and applications.
